Using infrared, internet and cellular technology, patient information can be downloaded directly onto Palm®, Pocket PC, Windows Mobile® and Blackberry® devices from the hospital’s information systems without disruption or custom modification.
CareFocus helps identify at-risk patient populations by automatically identifying and managing hospital or clinician-defined patient populations. Custom queries against the hospital patient data create target lists of high-risk patients according to lab values, risk factors for different conditions, age, gender, prescribed drugs and more.
The CareFocus profiling engine also helps protect communities by detecting possible disease outbreaks, bioterrorism threats and high-risk patient populations in real-time. Our solution searches all clinical data in all care settings across the health system, to track down patients that fall through the cracks of other monitoring systems.
