Enter information manually for Part II and Part III of Form 1095-C

Your client's medical benefit provider gives the information to enter in Parts II and III of Form 1095-C. We recommend importing this information using a 1095-C template spreadsheet. You can send the template spreadsheet to your client's benefit provider to get Form 1095-C and then import it into Accounting CS.
However, you can also enter Form 1095-C information manually by following these steps.
  1. Select
    Setup
    ,
    Employees
    , then the
    Personal
    tab.
  2. If the same information applies to multiple employees, select
    Edit Multiple Employees
    and follow the steps in the wizard.
  3. Select
    1095-C Part II: Offer and Coverage
    in the Affordable Care Act Information section.
  4. Select the reporting year, Box 14 coverage code, enter the Box 15 premium amount, and select the Box 16 Safe Harbor code.
    If any of the information changes from month to month, select Options to open the January - December Monthly Information window and enter the monthly information.
  5. Select
    OK
    .
  6. If the employer gave self-insured coverage during the reporting year, select
    1095-C Part III: Self-Insured
    .
  7. For each individual covered under the self-insured employee's insurance plan:
    1. Select the reporting year.
    2. Enter the individual's information.
    3. Mark the checkboxes indicating the coverage availability for the year.
  8. Select
    OK
    to save your changes.
  9. After you've entered the 1095-C Part II and Part III information, follow these steps to make sure your clients are set to process the forms.
  10. Select
    Setup
    ,
    Clients
    , then the
    Payroll Taxes
    tab.
  11. Select the client, then
    Edit
    .
  12. Select
    Additional Information
    in the Forms section of the Form 1095-C grid.
  13. Select a taxpayer and contact for Form 1094-C.
  14. Select the healthcare plan start month.
  15. Select the filing method for the Agency Copy/1094-C and Employee Copy.
  16. Clear the
    Do Not File
    checkbox, then select
    Enter
    .
