  • Boxes 3, 4, and 6 are entered in the UltraTax CS Source Data Entry Utility only, if applicable, and transferred to the input screen for informational purposes. Further research by the preparer will be required to determine if the amounts in these boxes have any tax consequences for the current or prior tax years.
  • To determine whether the amount in box 5 is taxable, refer to Publication 970, Tax Benefits for Education. If the amount is determined to be taxable (and it wasn't reported on Form W-2), open the
    Income
    screen in the
    Income
    folder, open the
    Other income statement
    window, then enter code
    5 - Txb scholarship not on W2
    in the Federal Code column. Then, enter an amount in the Amount column. This amount will appear on Form 1040, line 7.
