Overview
Use this screen to calculate Form 3552, Identity Theft Affidavit. Information transfers to this screen from the federal IDTheft Screen. A separate unit of the screen may be completed for each spouse if necessary.
To print Form 3552, you must enter data in the
Taxpayer received notice from FTB
field and you must also enter data in either the Explanation of tax impact on identity theft
statement dialog or in the Future impact

Per Form 3552 instructions:
- If taxpayer received a notice from FTB, mail Form 3552 to the address contained in the notice.
- If taxpayer did not receive a notice from FTB, mail Form 3552 to the address contained in the Form 3552 instructions.

is entered in this field, UltraTax CS prints a mailing slipsheet on the premise that Form 3552 is to be mailed to the address contained in the Form 3552 instructions.This field is required data entry in order to print Form 3552.
