Enter the date the business was discontinued or sold in the proper field if this is the corporation’s final return for the State of Illinois. The application enters

 in the Final Return checkbox on the form when a date is entered in the Date business was discontinued field.
Enter the FEIN of the parent corporation if this is a member of a group filing a federal consolidated return. If an Illinois combined entity type, the application enters the FEIN of the federal parent corporation.
Enter the city, state, and ZIP Code where accounting records are kept. If these fields are blank, the application defaults to the city, state, and ZIP Code in the client’s address.
