General Information
Designated agent information, if different from first member: Name
Enter the name of the designated agent if different from the 1st member. Per the Schedule UB instructions, the designated agent is a member of the unitary business group that must be an Illinois taxpayer and authorized to file the combined return as the agent for the other members.

Enter the FEIN of the designated agent if different from the 1st member.
Last year’s designated agent information, if different from current year: Name
If last year’s designated agent of the unitary business group is different from the current year’s designated agent, enter last year’s designated agent here. Per the Schedule UB instructions, the designated agent is a member of the unitary business group that must be an Illinois taxpayer and approved to file the combined return as the agent for the other members.

If last year’s designated agent of the unitary business group is different from the current year’s designated agent, enter the FEIN of last year’s designated agent.
Controlling corporation information, if different from designated agent: Name
If the controlling corporation of the unitary business group is different from the designated agent of the unitary business group, enter the name of the controlling corporation. Per the Schedule UB instructions, the controlling corporation is the corporation that directly or indirectly owns a controlling interest in all members of the unitary business group.

If the controlling corporation of the unitary business group is different from the designated agent of the unitary business group, enter the FEIN of the controlling corporation.
Mergers with members of the unitary group
Information entered in this statement is used to complete Schedule UB, Step 1, Section C. In the 1st column of this statement window, enter the name of each person who merged with any member of the unitary group during this tax year. Enter that person’s FEIN in the 2nd column of the statement window. In the 3rd column, enter the name of the member listed in Section B of Schedule UB with whom the person in column 1 merged. Enter the member’s FEIN in column 4.

Information entered in this statement is used to complete Schedule UB, Step 5. Enter in this statement window the member’s name, FEIN, and reason for exclusion.
